Rates and Insurance

The usual and customary rate for my practice is $150.00 - based on a 50 minute clinical hour. Therapeutic sessions longer tha nthis may be subject to additional service fees. Phone consults are billed at the same rate  propated according ly after 15 minites.

        - Medicare and Medicaid on a limited basis      - United Behavioral Healthcare

        - Aetna                                                                  - UMR

EAP services through:

        - APS                           - Cascade

        - ComPsych                - Magellan

Payment for services due each session unless arranged differently with me.

If using insurance, you are responsible for contacting your carrier for pertinent information prior first schedule session (Authorization & Number, Co-pay, Dedictible amounts that may imapct coverage).

Cancelation Policy: Appointment not cancelled or reschduled within 24 hours of schedule time will be billed at $ 75.00.

Payment: Visa, Master Card, and checks are accepted. Payment is due at time of services; if using inrance  co-pay is due each session
